Medical Escort Services
When a journey needs clinical care, your Medical Escort brings it along - across Canada and around the world.

A Medical Escort is a regulated healthcare professional who provides clinical care throughout the journey, on commercial flights and ground transport, for a traveller whose condition needs clinical care or monitoring for the trip to be safe. Portage plans and manages the entire journey behind the scenes, while your Medical Escort delivers hands-on clinical care from pickup to destination.
Who is this for
Our Medical Escorts support travellers whose condition needs clinical care or monitoring during the journey, from stable travellers who simply need clinical supervision to those with more complex needs. Common situations include:
Travellers leaving hospital, for home or for a hospital or care facility closer to home
Travellers moving between hospitals, rehabilitation centres, or long-term care facilities
Travellers who require oxygen or medical equipment during the journey
Travellers who need medication administration or clinical monitoring in transit
Stabilized travellers who need clinical supervision for the journey itself
If practical, personal support is all the journey needs, a Travel Companion may be the right fit.
How a Medical Escort journey works
Behind every trip are two roles: a Portage Case Coordinator, who plans the journey and manages the logistics, and your Medical Escort, the clinical front line, focused entirely on the traveller's care.
Before departure
Every journey begins with a clinical assessment of the traveller's needs. From there, your Case Coordinator books the flights and manages the logistics, including airline medical clearance, required equipment, seating and oxygen arrangements, routing, and timing, while our team plans for the travel environment and for situations that could arise en route.
Travel day
Your Medical Escort meets the traveller at the hospital bedside, at home, or at a care facility, and manages their care through every step of departure: ground transport, check-in, security, and boarding. Throughout the journey, your Medical Escort provides the clinical care the trip was planned around, from monitoring and administering medications to managing medical equipment, and updates the family and the care team at agreed points. If plans change mid-journey, your Medical Escort manages the traveller's care while your Case Coordinator handles rebookings behind the scenes.
On arrival
Your Medical Escort stays with the traveller until the journey is complete, whether that is admission at a receiving facility, settling in at home, or handover to family or a care team.
Medical Escort journeys are staffed by qualified clinical professionals matched to the case, including Registered Nurses and Advanced Care Paramedics, each with extensive experience caring for medically complex patients and prepared for the realities of delivering care in transit. Every journey is planned and delivered with appropriate clinical oversight. We work closely with hospitals, discharge planners, insurers, assistance companies, case managers, and families, so everyone understands the travel plan before departure.
Who are the Medical Escorts?
Commercial travel is what we specialize in
Many people assume a medically complex traveller automatically needs an air ambulance. With the right planning, equipment, and clinical support, many travellers can travel safely on a scheduled commercial flight, and that is what Portage specializes in: a safe, practical, and significantly more cost-effective alternative to air ambulance transport.
Whether a particular traveller can be safely managed on a commercial flight is answered through our intake process, before anything is booked. If the answer is no, we'll tell you and advise on the best way to get the traveller where they need to be.
When a commercial airline cabin isn't the right setting
Some travellers are stable enough to travel but are not well suited to a commercial aircraft cabin. For them, ground transport with the same clinical care is often the right answer: shorter journeys are routine, and longer ground journeys, such as Calgary to Winnipeg, are arranged when ground is the appropriate way to tr

Discretion throughout the journey
Your Medical Escort keeps a professional, low-profile presence from pickup to arrival. We arrange care to draw as little attention as possible: no visible medical branding, no more equipment on display than the journey requires, and any care given in the cabin provided as privately as the setting allows. Information about the traveller's condition is shared only with the people who need it to do their part.
Medical Escort questions
-
Our assessment is based on clinical care needs, not on how frail the traveller is. If practical, personal support is all a journey requires, we'll recommend a Travel Companion. Read our guide: Travel Companion vs Medical Escort: What's the Difference?
-
Often, yes. Airlines have medical clearance processes for travellers with clinical needs, and your Case Coordinator manages that clearance, the equipment arrangements, and the airline's requirements as part of planning the journey.
-
We assess this during intake, before anything is booked. For shorter journeys, ground transport with the same clinical care is often a practical alternative, and longer ground journeys are arranged when that is the right choice. Portage specializes in commercial airline travel and ground transfers, so if a traveller's condition calls for an air ambulance, we will tell you and point you toward the right provider.
-
When commercial travel is appropriate, a Medical Escort on a scheduled flight is significantly more cost-effective than an air ambulance. Every journey is quoted individually, based on the itinerary and the level of clinical support required.
-
Yes. Journeys between hospitals, rehabilitation centres, and long-term care facilities are among the most common trips we make, within a province, across Canada, or internationally. Not every transfer involves complex care; many travellers are stable and simply need clinical supervision for the journey.
-
Yes. Portage supports journeys within Canada, to and from the United States and Mexico, and worldwide, including travellers coming home, or moving to care closer to home, after an illness or injury abroad.
